Make an Accommodations Request

The Americans with Disabilities Act is a federal civil rights law that provides people with disabilities equal access to employment (Title I), state and local government programs (Title II), and goods and services (Title III). The City of Pittsburgh is committed to making City programs, services, and facilities accessible to all people. 

The City will not place a surcharge on any disabled person or group to cover the cost of auxiliary aids/services or other reasonable accommodations. 

ADA Accommodation Request Form

This form is only for accommodation requests pursuant to Title II - Public Services of the ADA. Please note:

  1. City employees seeking workplace accommodations pursuant to Title I of the ADA should contact Human Resources
  2. If you have previously requested and believe you have been wrongly denied accommodations under Title II, please file a grievance with the Office of Equal Protection.
  3. Individuals seeking accommodations from a private business or organization pursuant to Title III of the ADA should contact the business or organization from which accommodations are sought. 
  4. If you believe that your right to reasonable accommodations has been violated under any title of the ADA, please contact the City’s Commission on Human Relations.
  5. The City's non-emergency 311 Response Center manages requests regarding accessible parking spaces/signs. Please Dial 3-1-1 or 412-255-2621. The operator will ask a few questions to determine eligibility to apply for an accessible residential parking space and share next steps.
  6. If you need an accommodation for curbside refuse/recycling, please complete the City of Pittsburgh's Environmental Services's Medical Exemption paperwork.
  7. If you need assistance with snow removal or grass cutting, please apply for the Snow Angels or City Cuts programs.

Once you have done so, please hit “submit.” If a required field does not apply, please enter “N/A.” If you do not know the information under required filed, please enter “unknown.” If this form is inaccessible to you due to disability, a reasonable alternative for submission will be made available upon request to ADA@pittsburghpa.gov.
